Serious Occurrences

 The following conditions constitute a Serious Occurrence:

·        the death of a child who received childcare at a childcare centre,

·        abuse, neglect or an allegation of abuse or neglect of a child while receiving childcare at a childcare centre,

·        a life-threatening injury to or a life-threatening illness of a child who receives childcare at a childcare centre,

·        an incident where a child who is receiving childcare at a childcare centre goes missing or is temporarily unsupervised, or

·        an unplanned disruption of the normal operations of a childcare centre that poses a risk to the health, safety or well-being of children receiving childcare at the childcare centre.

In the event of a Serious Occurrence, the following steps will be taken:

ü  Immediate attention is to be provided as required. Provide First Aid or call 911.

ü  Contact the parents involved.

ü  Ensure all persons having knowledge of the occurrence remain at the site until excused.

ü  Contact the Supervisor and the President of the Board of Directors of the Centre

ü  Staff will report the occurrence to the Supervisor or the person in charge immediately.

ü  Contact the Program Advisor at the Ministry of Education within 24 hours of the incident. For after-hours reporting, leave message with answering service at 613-239-4164

ü  Supervisor or Primary Educator Witness writes & signs preliminary report sending copy to Ministry of Education Program Advisor

ü  A follow-up is then to be conducted with parents, involved staff, the daycare.

ü  The Serious Occurrence Notification Form must be posted at the entrance to the Centre for a minimum of 10 business days in the event of a Serious Occurrence.
